In the upper left of the sheet below each other must be specified:
-the name of the organization;
-the document type;
-date of the document;
-the registration number of the document;
-the city of creation or publication of the document;
-title to the text.

The basis for entering changes into the order (paragraph, points of order), a brief account of the circumstances which became the reasons for the publication of the order on changes.

The instructions to the officials that need to be made in connection with the amendment of the previous order (paragraph, points of order) and the publication of the new.

The item or items of the orderwho have lost legal force, in connection with the publication of a new order.

Execution of orders on amendments to previously issued orders or items made on sheets of A4 paper in accordance with the requirements of GOST P6.30-2003.
