Take the original document and select the fragments to be copied and transferred to the extract. Accurate quoting - the main requirement to the preparation of the statement. A certain form to such documents do not exist here, please refer to the General administration rules governing the registration of the business documentinformation. Much more important is the content of the statement, which must be composed of units of the required information.

To start, place the top center of the sheet the name of the document "extract from ..." (order, Protocol, etc.) and indicate briefly the subject, the essence of the document. Next, highlight and copy the part of the main document, which presented the initial details (company name, date and place of the event, number of participants, etc.). The introductory part of the statement is completed by placing a piece of text relating specifically to a given question (item of the agenda of the meeting etc.), with indication of its serial number according to the position in the source document.

Now find and copy a piece of text describing the discussion of this question (if it was) and taken a decision on it. Insert a quote in an extract, do not forget to include the numbering from the main document. Here, place names, initials and positions of responsible persons who signed the original.

Prepared statement reassure the persons authorized to execute such actions. Most often it is a Secretary or personnel officer. Here, the statement will consult the original document for a subject exact match. In conclusion, the statement should be written the word "True", signature (with interpretation in parentheses) of the responsible person, given his position, date and time of the certification document.

An extract from the document will need to be certified with the signature and seal of the organization in case she needs to be given to a third party.
