To make the title sheet, you'll need a sheet of blank A4 paper.
The top of the page, aligning the text center horizontally, write the name of the parent organization, the full name of the organization which done the report, a structural unit. Use for writing bold, Times New Roman, 14 pt.
In the center of the page (vertically and horizontally) write the word "Report". Use bold font to 20 pt., Times New Roman.

On the next line write "on the subject ____", putting the subject on which the report is written.

The following line should contain the text "on". The font size for the writing subject and the line – 12 pt.

Directly below, write the subject of the report without the quotes. Wherever possible, it should be as informative as possible, that is, fully disclose the contents you have prepared the report. Try without special need not to use the title of the report abbreviations. The theme of the report should also be written in bold, Times New Roman, type size – 16-18 pt. Don't make the font neither too large nor too small.
Conceding 1-2 rows in the worksheet to the right to specify the surname and initials of the author of the report. In this case, use direct, Times New Roman, size 12 pt.
Be sure to indicate occupied by the Rapporteur's position, academic degree. If the report is done by the student's secondary school, you can write "student __ grade". Use the same font and size as when writing the names of the Rapporteur. You can highlight the post in italics.
If the authors of the report is a group of authors, list them in alphabetical order. In this case, the first is often indicative of a person that is directly making the report.
Also, bellow you can specify the name and initials of the teacher (teachers) on the subject, which is the report.
At the bottom of the page, centered on two lines, be sure to specify the name of the settlement and the year when the report was read. The font for this action direct, Times New Roman, 12 pt.
