Find out the address of residence and phone you are interested in archive. Call today to find out about hours of reception, and the set of necessary documents.

State the problem and make an appointment, if the archive is located in your community.
If the archive is located in another city or another country, please request a certificate from the archive by registered letter with acknowledgment, to be sure that it will not get lost in transit.

In the preparation of the request to send in the mail include your full name. Write the reason for the request, for example: "I (name) ask You to send me the archived certificate (specify what kind) for lost (or write another reason). Mark the date and the painting.

Your request must include a copy of your passport or any other identification document, and a document indicating your participation in this certificate.

In making the Declaration at a reception in the archive to keep your passport (original) or power of attorney (original), if an application executes another person. And a document proving your involvement in the purchase of this document.

Fill out the application form on the official letterhead or arbitrarily.

In the statement and request be sure to include your contact phone numbers. After completing the form, save the coupon issued to employees of the archiveprior to issuance of certificate. The certificate will be issued within 30 calendar days from the date of application.
