Set record ordinal number (column 1). In column 2 put the current date. Write it in Arabic numbers, in this order: number – 2 characters, month – 2 digits year – 4 digits. For example, 01.08.2011. Use this format when you specify any date in the workbook.
In column 3 "Data on employment, transfer to another permanent job, qualification, dismissal," email "was Adopted (adopted) to work part-time...". Next, specify the name of the employer, name of Department and position. In column 4 specify the document under which the employee is enrolled in the state. The name must be exact and complete, showing the date of the document and its serial numbers. At first, specify the date and document number and then the name.
At dismissal in column 3 make the following entry: "Dismissed (fired) from work for part-time..." , then specify the name of the employer, then the law article, part, paragraph, in accordance with which occurred a termination of employment. Also need the exact transcript of the law.
When a worker is working part-time in the same organization where he is busy on a primary place of employment, record of employment for part-time name of employer not specified.
If the employee was employed on a part-time, and a contract terminated by the time of entry in the labour book, in column 3, write: "From (date) to (date) we work (worked) for part-time..." then specify the name of the employer, name of Department and position. Although the record is about the last period, the date in column 2, however, needs to be current.
When an employee with multiple positions transferred to main place of work in the same organization, the first make entry of dismissal (see paragraph 3). The second record run: "the Accepted (accepted) for the position (specify job title)".
