Before completing the form carefully read the rules. Note that these are only the cases with full author name and email address.

In the appropriate fields write your surname, name and patronymic name, your address, the location of the institution - the name of the region, territory or Republic. Also don't forget to include your email address. Specify the topic of the treatment on the quality of goods, on health problem or the other.

Fill in the form intended for the text of the appeal. It should be the name of the organization that sold you a defective product or provided inappropriate the stated level of service, as well as the complete address of this company. State the facts clearly, if you know what specific legislation has been violated by the seller, please indicate this in the letter.

After filling the form click "Send". If the upload was successful, your email address will receive a confirmation about it.

If you want, you can transfer your claim in person at your local branch of Rospotrebnadzor. To do this, write a letter, do not forget to specify your surname, name, patronymic and address, and coordinates the enterprise - object of the complaints. The application is written in free form. Then find the address of your regional office of Rospotrebnadzor. On the organization's website go to "Control in the regions". You will see a list of regions, and addresses.
