Before making an entry in the work book, you need to make the order of dismissal of the employee, which must be signed by the employer and accordingly the employee.

In the order specify the reason for the dismissal. The labour code offers employers several dozen formulations. The most common are: dismissal in connection with the expiration of the employment contract on the initiative of the employee in connection with the transfer to another organization in connection with the violation of worker rules under the Labour code, in connection with the reduction and others.

On the basis of this order and do the entry in the workbook. Remember that the date of dismissal is the date of signing of the order, respectively, the entry must also be of this number. This document is filled carefully and with the use of ball point or gel pens of blue or black. Abbreviations are not allowed.

Then proceed to fill books. In the field "number of entries" put the number that follows the previous record. The date should be written in this format - DD.mm.yyyy. In the "work experience" you must specify a reason for the dismissal, citing article. For example, if resignation is the entry: "Dismissed at his own request, paragraph 3 of part one of article 77 of the Labour code of the Russian Federation". When dismissing an employee in connection with the termination of the employment contract in the labour book is written: "Dismissed in connection with the termination of the employment agreement, paragraph 2 of article 77 of the Labour code of the Russian Federation". If the employee is transferred to work in another organization, the entry would be: "Dismissed in connection with transfer at work in the (name of organization), item 5 of part one of article 77 of the Labour code of the Russian Federation".

Then in the "document Name" you must specify the order number and the date of preparation. Then sign this record signature and seal of the head of the organization. Don't forget to write the information on entering of record in the labor book in a personal card of the employee where he needs to sign, signifying his assent to any modifications.
