Submit your request for deregistration to the Federal Migration Service or in the housing office (this will depend on your place of residence). By the way, the statement can be handwritten and submitted in electronic form. Additionally, provide your passport, which will bear the appropriate seal. Expect the finished documents will have a short time you will receive them in three days.

Together with all the documents you receive and the piece of departure. In any case, don't lose it-you'll need it when making a new residence. However, if the leaf will still be lost when registering at the new place of residence will notify about this in written form. But don't worry, its absence may be cause for rejecting your application.

You can do another thing: after moving to a new residence, apply simultaneously the two statements. One of them will have information on registration at a different address, and the second, on the contrary, the removal from the register. The second paper submitted to the territorial branch of the migration service, located at the old address. And your passport will immediately appear two punch: and the decree on the registration account and the Bank statement of the last housing.
Note
There are situations in which an extract from the apartment are not required to submit a statement and report to the appropriate authorities. These include, for example, military service (statement made just on the basis of reports from the military). In addition, the deregistration can be made on the basis of a court decision (for example, if the residence permit presented false documents).
