At the top of the A4 sheet on the center line enter the full name of the organization under the Charter of the company. If the company has an approved form for business letters and documents – use it by copying the "hat". If necessary, write down the law and fact- diffraction address, phone number, Fax, e-mail.

On a new line write the name of the letters "Act No. __ refusal of receipt". Below, specify the location and date of the: city, date, month and current year.

In the text of the actand record the surname, initials and position of the originator and provide information about the involved witnesses. Their role can be staff, service personnel or employees of the unit, which is the person who does not accept the documents. For drawing up the actand bring at least two witnesses, and in order to avoid unpleasant proceedings with the Commission to resolve labor disputes choose for the role of disinterested witnesses and informed persons.

Specify the transfer date of the document in his hands. If necessary, specify the time and place of action. List the name, number, date of publication, which refuses to accept the employee. Specify the documents, to make demands of transfer orders, approved standards, orders, articles of organization, etc. will be reflected in the acte failure verbal explanation of the employee. If he refuses to receive the document without explanation, make an appropriate note.

If necessary, describe further manipulation of the document, which remained in the hands of personnel officers: how and where the instance will be stored, how it will be sent by mail, who will be responsible for further storage and possible transfer to third parties.

Signature of the preparer and witness preparation. Decipher the initials and titles. If the employee is not receiving the documents refuses to sign this act, make a note and ascertain his signature and the signature of witnesses.
